Target Audience

Family Physicians, Internal Medicine Physicians, and other healthcare providers with an interest in caring for patients with diabetes.

Learning Objectives

  1. Create patient-centered management plans for patients with diabetes focused on reducing complications through the ABCS – A1c, blood pressure, cholesterol, and smoking cessation
  2. Follow evidence based guidelines for the care of patients with diabetes
  3. Recognize the importance of treating to patient-centered goals and act to intensify therapies when warranted
  4. Recognize patient, physician and system barriers to medication use and take action to mitigate them
  5. Counsel patients on the importance of physical activity, weight management, and healthy lifestyle choices in the management of diabetes
  6. Implement diabetes self-management plans, patient education, and patient engagement tools to involve patients in their care
