Aledade to Form Primary Care Physician-Led ACO in New Jersey

Leader in Value-Based Care Working with the New Jersey Academy of Family Physicians to Recruit Independent Primary Care Practices Across the State

In New Jersey, Aledade will work with the NJAFP to achieve the shared goal of helping primary care physicians embrace practice innovations and transformation to achieve the best quality care. Aledade has already recruited local, independent primary care practices to join the statewide ACO and will continue to add practices as it works toward a January 2018 launch. While awareness of issues surrounding adult immunizations have increased in New Jersey, our rates lag behind recommended targets and remain lower than in most other states. Members of the New Jersey Immunization Network (NJIN) are asking you to complete a short survey that will help determine NJIN priorities, emerging needs and challenges, and help shape the agenda for the 2018 Adult Immunization Congress. Updates on New Opioid Prescribing and Resources 

The State has created a webpage with all the information you need on the new opioid prescribing laws, as well as links to key resources including a sample one-page pain management agreement and FAQs on the law.

IMPROVEMENTS ANNOUNCED TO THE PMP

  • NJPMP’s Interstate Data Sharing now covers 12 states, including PA and NY.        
  • Expands patient history searches to two years of patient’s prescription records.
  • Automatically converts dosages of commonly-prescribed opioids of differing potency, such as codeine, fentanyl, and oxycodone, into a standard value known as “morphine milligram equivalents” (MME).
